Chronic care management after regenerative treatment is a monthly check-in from the Regen.MD care team in St. Louis that carries you through the months your tissue needs to heal. It is offered to patients who also live with long-term conditions such as diabetes, high blood pressure or chronic pain, because those conditions shape how well a PRP or bone marrow treatment takes.
The injection takes an afternoon. The repair it starts takes months. Tendon, ligament and cartilage have a thin blood supply, so they rebuild slowly, and what you do with the joint in month two and month four matters as much as what went into it on day one.
Healing runs on a calendar, not a single visit
An orthobiologic treatment sets off a repair response: cleanup first, then new collagen laid down, then that collagen slowly lining up along the lines of stress the joint actually sees. That last stage is the long one. Fibers that are loaded gradually line up strong. Fibers that are rested too long, or overloaded too soon, line up weak.
The trouble is that the joint usually feels better well before it is finished. That is the month people go back to full tennis, a long hike or heavy yard work, and the tissue is not ready. A monthly call catches that moment and steers you back to the plan your physician wrote. How our orthobiologic treatments work explains what the injection itself does.
Loading the joint: the plan you carry home
Every patient leaves with an activity and loading plan: which movements to do, how many, how often, and when each one steps up. Most of it is ordinary work, such as walking, controlled strength exercises and exercise band work, adjusted week by week.
Each month your care manager asks what you did, what hurt and for how long, and whether the next step felt too easy or too hard. Pain that keeps building from one day to the next is something your physician needs to hear about, and anything clinical reaches your physician the same day.
Blood sugar and inflammation decide what the cells can build
Repair is construction work, and construction needs a clean site. High blood sugar stiffens new collagen by sticking to it, and it slows the small blood vessels that bring in supplies. Belly fat and a diet heavy in sugar and seed oils keep the whole body in a low, steady state of inflammation, which tells repair cells to keep cleaning up instead of building. Short sleep and constant stress add stress hormones that break tissue down.
That is why the monthly contact covers your sugars, your weight, your meals and your sleep alongside the joint. Where a home glucose meter, pressure cuff or scale is part of your plan, the numbers flow to your care team on their own, a staff member goes through them on fixed days, and anything outside your target reaches your physician before the day ends. Who joins, and what the program asks of you
Patients qualify when they carry at least two long-term diagnoses alongside the joint or tendon problem: blood sugar trouble, high blood pressure, nerve damage, weak circulation, ongoing pain, excess weight, depression or sleep apnea. A single serious diagnosis can anchor a narrower version. For pain that stands alone, your physician offers a chronic pain program and runs it in person.
Sign-up happens at an appointment with the physician who did your treatment. You consent, spoken or signed, and walk out with goals written the way you said them, kneeling in the garden again, for instance. Every month a nurse or medical assistant gets in touch by phone, video or portal message, so recovery check-ins never cost you a drive. The physician reads each report and keeps sole authority over prescriptions, while the care manager runs down labs, referrals and forms. When stress or low mood stalls the rehab, our in-house psychologist adds Acceptance and Commitment Therapy on the same monthly beat.
Your plan is billed for it monthly, and your coverage may leave you a copay or coinsurance; we confirm coverage and explain your share before enrollment. One practice per month is the limit, so tell us if another clinic already checks in on you each month. You are free to leave the program any time.
The monthly contact is not an emergency line. Call 911 for chest pain, fainting, labored breathing, sudden slurred speech or one-sided weakness, or if you are sedated to the point of being hard to wake.
Frequently asked questions
Do I need chronic care management to have PRP or BMAC?
No. It is a separate service for patients who also have long-term conditions, and you can decline it or stop it at any time. To see whether a regenerative treatment fits your joint, start with am I a candidate for regenerative treatment.
My knee feels great after six weeks. Can I skip the rest of the plan?
Feeling better comes before the tissue is finished. Keep stepping up the load on schedule and report how it goes on your monthly call. For the knee in particular, see PRP knee injection in St. Louis.
What if the treatment does not seem to be working?
Say so on the call. Your physician reviews it and decides on the next step, which may be a change in the plan rather than another injection. Our page on when orthobiologics are not the right treatment covers how we decide.
