This is not a contact form, and this is not a sales consultation. It is the first step of a clinical investigation. Evaluation is contingent upon review of your metabolic and structural data.

A comprehensive evaluation — not an appointment slot.
Assembly-line clinics offer consultations that are sales pitches. Ours is not. Before you arrive, a physician has already reviewed your history, imaging, and metabolic data, and the session that follows is a comprehensive evaluation of that data.
The diagnosis and the intervention are two distinct clinical events. The evaluation decides whether any intervention is indicated at all.
Initial Clinical Evaluation & Data Review
- Professional review of your medical history, imaging, and metabolic data prior to the session.
- A comprehensive 60-minute physician-led discussion of the root cause of your pain.
- A written roadmap for your clinical terrain — the bottlenecks, the sequence, and the instruments indicated.
We only accept patients who provide the required labs and imaging before this evaluation. If you cannot commit to your appointment time, you are not a candidate for a terrain-based recovery protocol.
What the application requires
Prepare these five items. Incomplete applications are returned with an explanation of what is missing and why it is required — we cannot evaluate a terrain we cannot see.
